The product, in the report: Navien Tankless Hot Water Heater

Description of Product Safety Hazard: On October 2, 2026, my Navien NPE-240A2 natural gas tankless water heater (Serial Number: 2087B2212451312) suffered a sudden failure of the primary heat exchanger. Upon reporting the failure, Navien corporate claims representative [REDACTED] issued an official corporate determination stating that the unit failed due to the “ingestion of acidic exhaust gas” (flue-gas recirculation). This official corporate finding by the manufacturer indicates a severe mechanical design defect or component malfunction that allows dangerous combustion exhaust products to short-circuit back into the appliance intake. The recirculation and ingestion of toxic, acidic flue gases inside a gas-fired residential appliance pose an immediate hazard of carbon monoxide (CO) poisoning, indoor air contamination, and potential fire or explosion due to compromised internal combustion. Furthermore, Navien is making these critical safety determinations regarding dangerous exhaust gas ingestion solely by visually guessing from photographs, refusing to deploy qualified field technicians to measure, test, or physically inspect the unit for widespread structural or component hazards. This unit has been replaced out of necessity to mitigate the immediate safety threat to my household. A full technical demand letter detailing the system failure is attached for the Commission’s review.

We began to smell an intense odor coming front he exhaust tanks outside last night, 9/16/2015. After calling Columbia Gas they detected a 58pmm reading of carbon monoxide being emitted from the exhaust system which was leaking into the home. All gas was turned off and the account was “red flagged”. Larry and Sons came out today and informed us the unit was actually installed years AFTER it was federally recalled. At this point I am waiting on how they are going to resolve the issue. Thank you.

Was notified by local gas service provider that the installed Navien natural gas tankless water heater was venting unspent natural gas. The technician informed me that he was there due to a complaint of the strong odor of natural gas in the area. Upon further investigation the technician stated that the unit was venting unspent natural gas well over the LEL (Lower Explosive Limit).

Unit leaked water from a defective elbow in attic. unit does not light due to defective flow sensor. I put the approximate date of the leak. The unit has had constant issues. Usually the flow sensor for the water. the company said the unit had a recall but was only willing to pay 100.00 of the cost of the repair. Parts are still setting in my garage as the plumber wants another 200+ to install the collar for the gas outlet pipe. Why should I have to pay to fix a know manufacturing defect. The company refuses to stand behind their product and expects the consumer to pay for their manufacturing and engineering shortfalls

The product, in the report: Navien NG-210A-NG Tankless Water Heater

I have a Navien model NG-210A-NG that has been very unreliable over the approximately 3 years it has been installed, including many cold showers. The installer, Ballard Natural Gas, has been out several times replacing various parts under warranty.

Yesterday, the issue went from a reliability nuisance to a serious safety issue. After hearing noises outside, I went to investigate to see clouds of smoke and flames coming out of the Navien exhaust PVC pipe. I have attached a photo of the exhaust pipe where you can clearly see the PVC has melted from the flames. I feel fortunate I was home and able to quickly shut off the gas supply before my house, or my neighbor’s house, caught fire or anyone was hurt.

The product, in the report: Navien Tankless Water Heater

The exhaust vent collar was replaced on my Navien tankless Water Heater on March 6th per recall Release #12-074 due to the risk of an improper seal and subsequent carbon monoxide venting into the house. In my case the seal as installed was ineffective and exhaust was venting into the house. This was due to the Navien vent collar being made of nylon and being unable to be “welded” to the PVC pipe using the regular adhesive used in the plumbing (and other) trades.

A few weeks after replacement of the Navien exhaust vent collar it was noticed that the Vent Terminator had fallen out of the vent pipe to the ground about 15 to 20 feet below. This was likely due to the increased vent flow (now that it was venting correctly) plus the fact that the Vent Terminator, supplied by Navien as part of the Water Heater installation kit, is made of the same type of nylon material as the original exhaust vent collar covered under the above referenced recall. It is apparent that the Vent Terminator had been installed using the same regular adhesive used in the plumbing (and other) trades for PVC fittings. It was not apparent from any Navien documentation (nor visually at the time of installation) that the Vent Terminator was made of nylon rather than PVC. Also, Navien did not provide any alternative adhesive in the installation kit nor information on what should be used as an adhesive between nylon and PVC.

The Navien provided Vent Terminators are intended to provide a “screen” over the ends of the Water Heater exhaust and inlet vents. Without a Vent Terminator installed on the exhaust vent pipe there is the likelihood that birds or other animals can cause a blockage that may result in carbon monoxide venting back into the house where the Water Heater is installed (in our case). This potential unsafe condition is very similar to the one covered under the current recall notice referenced above.

Should the scope of the recall be expanded to cover this issue?

In my communications directly with Navien they have not taken any responsibility for the issue and they refuse to see it as “their problem.”
