The product, in the report: glass insulated bottle

Christmas, 2014, I was given the gift of an “glass insulated bottle” and used it off and on over the past 12 months, primarily to bring hot coffee to work. Yesterday, January 18, 2016, I tried to use the bottle at home for purposes of storing and serving cold water to a friend. I filled the thermos with tap water approximately half full and then proceeded to put ice cubes into the bottle. After putting in about a half dozen ice cubes, there was a loud bang (about as loud as a firecracker) and explosion. The glass lining imploded or exploded, sending small and large (inch or so) shards of glass out of the opening of the thermos. I was loading the ice cubes into bottle carefully, one by one, gazing directly into the opening, and it was extremely fortunate that none of the glass fragments hit my face. This seems like a very dangerous situation and I would request that your agency would do all that you can to warn other consumers about this danger and have the manufacturer and/or seller make changes in the design and/or production of this product or remove it from the market.

The description on the label described the product as “Item/Article No. 75TH.” The UPC was “8___50223 00016___1.”
