The product, in the report: Water bottle, no company information on product, bottom reads made in china and bpa free. Branded with P&G. Clear bottle with blue top and bottom. Twist off black lid tied to bottle with string.
2 year old daughter removed screw off cap from water bottle to drink water. A small rubber seal remained on the mouth of the water bottle. When daughter started drinking the rubber seal went into her mouth and she started choking. I have several pictures to show of product.
