The product, in the report: O2COOL® 2-Piece 30 oz. Protein Shaker Bottle in Blue The O2COOL 2-Piece 30 oz. Protein Shaker Bottle Set is your go-to mixing bottle to start your day off right Easy-to-open flip top Spiked mixing ball to freely move ingredients and break up clumps Rounded bottom eliminates collection points of unmixed powder BPA- and phthalate-free Plastic Dishwasher safe 30 oz. capacity Measures 3.63” L x 8.5” W x 9” H Imported SKU # 47195387

Product: O2COOL® 2-Piece 30 oz. Protein Shaker Bottle in Blue. Purchased from Bed Bath & Beyond SKU # 47195387

The protein shake was purchased at Bed Bath and Beyond on 2/17/2017. The shaker was dissembled and washed in warm soapy water before first use.

On first use, the shaker’s spiked mixing ball had one of the spikes break free. The spike was found in the mixed shake and could have been ingested. The spike was approximately 1 inch in length and 0.1 inch diameter.

Placing the ball back into the shaker after it was cleaned and shaking again (vigorously) another spike was broken from the mixing ball.

It appears that there is a fundamental design deficiency with the spiked mixing ball or a defect in its manufacture.

Made Nov 26, 2013, sent to the company Dec 5, 2013, published by CPSC Dec 27, 2013

The product, in the report: Blue sports drink bottle, with green mist spray lever.

I had bought this sports bottle from Academy Sports for my 2nd grade child. The bottle has a pull and push stopper that you drink through. After just two or three uses, while my 7 year old son was drinking out of the bottle, a small plastic piece came out of the stop center, causing my son to gag. Luckily he was able to cough and spit it out without causing any damage to himself or to any of my younger children.
