Made Jan 3, 2020, sent to the company Feb 3, 2020, published by CPSC Feb 18, 2020

The product, in the report: Zak Designs 17.6 oz water bottle. PJMasks blue and green design

Product: Zak Designs17.5 oz plastic water bottle. PJ masks design. Model PJMD-T090. Purchased at Target on 12-30-19.

During first use, my child dropped container fro height of approximately 3ft off floor. Container half full of water. When container struck floor, top portion of bottle broke off in the lid. Lid and threaded top portion of bottle completely separated. Separation caused extremely sharp, jagged edges in the plastic. My child received laceration to her hand when she picked up the pieces.

  2. First aid

Made Jun 27, 2016, sent to the company Jul 6, 2016, published by CPSC Jul 20, 2016

The product, in the report: Zak Designs 26oz water bottle sold exclusively at target June/July 2015.

We received the replacement lid from Zak Designs for the 26oz water Bottle Sold exclusively at target in June /July 2015. We have the Star Wars Design. As soon as I heard about the recall I immediately stopped using the original lid and ordered the replacement. Zak shipped the replacement in a timely manner and we began using it right away. I did think it was unusual that it didn’t come with the clear, soft attachment that you put your mouth on but just figured that was the new design.

Today my son came home from camp with the water bottle and said his throat hurt because something broke off the lid and he swallowed it. I looked at the mouthpiece and a small jagged black piece is missing. Probably a 1/4” in size.

I have requested yet another replacement from Zak but thought the CPSC should also be aware of this incident.

    Thank you.

The product, in the report: Zak brand Wonder Woman water bottle, purchased at Target.

I purchased my 6 year old daughter a Zak Designs brand Wonder Woman water bottle. She used it for several weeks. One evening, she was drinking water out of it and stopped suddenly. A piece of the inner straw had broken off and she had swallowed it. As it was sharp plastic, we took her to the emergency room where she was xrayed and stayed for several hours to make sure the sharp edge didn’t puncture her lower digestive tract. Thankfully, she was fine. I just received a call tonight from Target (where we purchased the bottle) and I checked their website to see what bottle they were talking about and saw it was the WonderWoman Zak brand bottle. I am VERY pleased to see the recall was issued. Extra kudos to Target for utilizing their computer/store Red Card systems to contact me to let me know about the recall. Unfortunately, for us it was a bit late, but again... we are VERY thankful the piece of plastic didn’t do any internal damage to our daughter.
