Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

  1. Emergency room

Made Jan 13, 2016, sent to the company Feb 11, 2016, published by CPSC Feb 26, 2016

The product, in the report: Cardboard Box with Plastic Handle (Packaging for a Loveseat)

The consumer was unloading the packaging from a pick-up truck. The tailgate was down.

The consumer was holding the packaging from a plastic handle in the back. There was a handle on the inner side and outer side of the box. Both of the handles gave away and separated from the cardboard box. This caused the consumer to lose her grip. It caused a shift in the box and the consumer crushed her left forearm in between the tail gate and the box.

The consumer went to Urgent Care. There was a fracture in her Ulna on her left arm. The consumer was provided a soft brace by Urgent Care. The consumer was referred to an Orthopedic Surgeon. Few days later, the consumer saw an Orthopedic Surgeon. He put her in a half cast.

On 12/23/15, the consumer was put in a soft brace again to allow some movement. They diagnosed that the fracture is not healing so the consumer has a surgery scheduled for 01/21/16 with the orthopedic surgeon.

The consumer contacted the manufacturer regarding the issue. The consumer spoke to the VP of Operations ([REDACTED]) at Emerald Home Furnishings. The consumer sent photos to [REDACTED]. The consumer was informed by him that he would send the photos and description of the incident to the insurance company. He contacted Alliance Global Corporate and Specialty and provided the adjuster’s information to the consumer.

The consumer contacted the adjuster directly after not hearing from anyone. The consumer spoke to [REDACTED] (Claims Adjuster II at Alliance Global Corporate and Specialty; Phone: 1-314-817-2967; Fax: 1-866-722-4317; Claim # [REDACTED]). She informed the consumer that she only received a copy of the receipt, description of the box and fuzzy picture from doctor’s report. The consumer sent all the files to her directly on 11/1/15.

The consumer sent an e-mail to Ms. [REDACTED] after not receiving anything from her after the information was sent to her. The consumer also called her supervisor, [REDACTED], after not hearing from her. The consumer received an e-mail from Ms. [REDACTED] on 11/18/15 who informed her that she was trying to get more information on the box.

On 12/8/15, the consumer contacted Ms.[REDACTED] informing her about the pending surgery and that she was still waiting on her case update. The consumer last contacted Ms. [REDACTED] through e-mail on 1/12/16. As of 1/13/16, the consumer has not heard from Ms. [REDACTED] on the status of her case. The consumer said that she has also tried to contact her through phone numerous times but no one calls her back after she leaves a message.

The consumer is not looking for compensation. She is looking to make sure that the company looks into the defect of the packaging so someone else does not suffer from a similar incident as her.

The consumer stated that there were no warnings on the box that can inform the consumer to handle with care in case the handle breaks.
