The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

  1. First aid

Made Jan 17, 2018, sent to the company May 8, 2018, published by CPSC May 22, 2018

The product, in the report: It is a packaging problem for a generic drug called Estradiol Vaginal Inserts, USP 10 meg. I receive this drug through a prescription with Express Scripts. It’s RX number is: [REDACTED] prescribed for me by [REDACTED] who is aware of this problem. It is used as a vaginal tablet insert to increase estrogen levels. The brand drug, Vagifem, did not have this foil package problem. The company that issues this product is: TEVA Women’s Health Research, 1090 Horsham Rd, North Wales, PA.19454. Tel: 888-TEVA-USA (888-838-2872) or (215) 591-3000.

The perforated foil packaging for the generic prescription drug, Estradiol vaginal inserts 1 0 msg, has sharp edges that break open midway at the perforation line separating each plunger insert, causing these broken edges to puncture fingers and hands and exposing the adjacent insert creating an unsanitary product. The perforation does not break away as it should when it separates each set of four vertical insert plunger tabs. I cut my finger on one of these broken perforated edges, causing bleeding, but no medical emergency. I am including photos of the tom foil packaging and the damage to my hand.

This generic product is provided by the Express Scripts home delivery prescription service of my insurance company, the American Foreign Service Protective Association (202) 833-4910. The company that Express Scripts uses for this generic is: TEVA Women’s Health Research in North Wales, PA. {215) 591-3000. The phone number for Express Scripts is (800) 818-6717.

Complaint summary: Generic packaging should not be so cheap and shoddy that it is dangerous and harms patients. Express Scripts/AFSPA should find another company that provides safer packaging for the generic if they no longer want to offer the original brand name product (Vagifem) which did not have this dangerous packaging problem.
