Seen by a doctor or nurse Made May 24, 2012, sent to the company Jun 26, 2012, published by CPSC Jul 11, 2012
The product, in the report: stainless steel thermal container, approx 9.5 inches tall, 2.5 inches wide. The Target receipt lists the following sku number 002100423
I purchased the thermal container in January 2011. In February 2011 my 10 year old daughter used it for the first time to drink hot tea. The top of the thermal container has a push valve that says open/close and that has holes. This looks like where you are supposed to drink from. But when she tried to drink, the tea came out from the sides of the thermal container lower down. She received severe burns to her chin and chest.
