What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 310 words
“Based on medical record review, observation, document review, and staff interview, the facility failed to ensure that an accurate drug profile was maintained in one (1) of fourteen (14) medical records reviewed (Patient #3). This failure has the potential to affect all patients receiving medication services through the facility. A medical record review was completed on 3/24/25. The medical record revealed there was no Synthroid on the current medication profile for Patient #3. An observation was completed on 3/25/25, at 8:45 a.m., with Staff #1 and Patient #3 during a home visit. During the observation, Staff #1 was reviewing Patient #3’s medication and it was discovered that the patient has been taking a Synthroid 50 mcg once a day that was not on the active medication list. When the patient was asked about the medication they stated, “My doctor put me on it.” Staff #1 asked Patient #3, “Didn’t we discuss discontinuing the Synthroid?” The patient stated, “Yea but the doctor put me back on it.” Staff stated, “we don’t have that medication on our list. I will add it now and let them know at the office.” A policy titled “;Patient Care Policies & Procedures” (Medication-Management), last revised 10/2018, was reviewed. The policy states in part, “;Policy...(5). A medication profile is maintained. For every patient and includes a listing of the current medication orders for each patient and specifies whether the medication is or is not related to the patient’s terminal illness. (6). The Medication profile includes all prescription medications, over the counter drugs, vitamins, and herbal and alternative supplements/remedies... (9). The Hospice monitors the medication dispensed to and used by the patient.” An interview was conducted on March 25, 2025, at 10:35 a.m. with Staff #2. Staff #2 stated, “;Patient #3 was not on Synthroid at admission. Synthroid was not on the active medication list until today, 3/25/25.””

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
