What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 291 words

“Based on record review and interview, the agency failed to provide spiritual counseling services at end-of-life for 1 of 2 patients (Patient #13) that passed away in a total of 14 patient records reviewed. Findings include: Review of agency policy titled “Hospice Services and Admission Policy” dated 10/09/2023 revealed “Purpose: To describe Hospice services provided by the agency. 3.1 Core Services: a Services are available 24 hours a day. g. Counseling.” Record review for Patient #13 revealed patient was admitted 07/23/2024 with terminal diagnosis of Acute and Chronic Respiratory Failure with Hypoxia. Initial MSW (Medical Social Worker) admission assessment dated 07/23/2024 revealed patient was not spiritual, was offered and declined Chaplain services. On 08/11/2024, Patient #13 passed away. Record review of SW “Collaboration Note” dated 08/08/2024 at 12:54 PM revealed “Chaplain Referral: Request for Spiritual Support. Referral being requested for: Patient and family. Spiritual beliefs: Upon admission this MSW was informed that they were not really religious and/or affiliated with any specific church but was informed by the nursing home SW that the wife was requesting spiritual support now that patient is declining. Specific Needs: Spiritual support as requested as patient is declining in condition. Wife has been sitting bedside and will be able to express any specific support needs.” Record review for Patient #3 revealed no spiritual support/counseling/chaplain services were provided from referral on 08/08/2024 to patient passing 08/11/2024. During an interview with Administrative Secretary D on 10/10/2024 at 1:40PM, the above findings were reviewed. Administrative Secretary D stated, “We cannot find documentation. The patient refused Chaplain initially and was not spiritual. We have a volunteer who is also a Chaplain and may have made a visit, but we have no documentation of that or why a visit wasn’t made.””

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
