What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Hospice aide assignments and duties (42 CFR 418.76(g)(1))
The inspectors’ words, 287 words
“It is required that hospice aides are assigned to a specific patient by a Registered Nurse who is a member of the Interdisciplinary Group. Written patient care instructions for a hospice aide are prepared by a RN who is responsible for the supervision of the hospice aide. This standard was not met as evidenced by clinical record review and document review. In 1 of 3 (33%) clinical records reviewed for patients that elected hospice aide services revealed the agency did not ensure the Registered Nurse (RN) provided the aide with clear and accurate patient-specific written care instructions on the aide care plan. CLINICAL RECORD 2: The interdisciplinary group (IDG) plan of care (POC) dated 01/08/2025 included a nursing order for wound care to the “Stage II Pressure Ulcer to Coccyx: Utilizing clean technique - cleanse with wound cleanser, cover with foam dressing and change every 3 days and prn. Instruct caregiver in wound care”. The aide care plan dated 01/06/2025 lacked evidence of clear patient-specific written instructions for how to manage and what to report related to the wound area on the patient’s “Coccyx” while providing the assigned task: “Bed bath”. The aide visit frequency ordered was 2 times per week. The aide visit notes dated 01/10/2025 lacked evidence of how the aide managed the “Coccyx” wound while performing the assigned task: “Bed bath.” The Executive Director, Director of Clinical Operations, and Director of Business Operations stated in an interview on 01/16/2025 that the agency did not ensure the hospice RN provided the aide with accurate and patient-specific written care instructions on the aide care plan related to how to manage and what to report related to the “Coccyx” wound while performing the ordered task: “Bed bath.””
