What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Content of plan of care (42 CFR 418.56(c))

    The inspectors’ words, 281 words

“It is required that each patient’s individualized written plan of care reflects patient and family goals, and planned interventions based on problems identified in the initial and updated comprehensive assessments. This standard was not met as evidenced by clinical record review, document review and interview. In 1 of 14 (7%) clinical records reviewed, the agency did not ensure the patient’s individualized written plan of care reflected patient and family goals and identified planned interventions based on problems identified in the initial and comprehensive assessments. Agency Policy #HC.31, Plan of Care (revision date 09/2024) read in part, “The plan of care specifies interventions, care and services necessary to meet the needs of the patient, family and/or caregiver identified in the comprehensive assessment and ongoing assessments of the patient that reflect patient and family-centered goals as well.” CLINICAL RECORD 2: The patient elected hospice care on 12/13/2024 with a hospice diagnosis of senile degeneration of the brain. The RN (Registered Nurse) Initial Assessment completed on 12/13/2024 included documentation that read in part, “When is the patient dyspneic or noticeably short of breath? With moderate exertion”. The Plan of Care Order, effective 12/13/2024 through 03/12/2025, included Oxygen 2 liters via nasal cannula as needed for shortness of breath. The medication profile effective 12/13/2024 and updated on 01/07/2025 included Oxygen 2 liters via nasal cannula as needed for shortness of breath. The individualized plan of care, effective 12/13/2024 to current, did not include any patient and family goals related to shortness of breath or the use of oxygen. The Administrator confirmed the findings in a joint record review and interview on 01/09/2025 and stated in part “I will enter patient and family goals for this.””

    Rule L0545. Standard-level.

  • Discharge or transfer of care (42 CFR 418.104(e)(2)(i))

    The inspectors’ words, 229 words

“It is required that if a patient revokes the election of hospice care or is discharged from hospice per hospice regulation §418.26 (i.e., no longer terminally ill), the hospice forwards to the patient’s attending physician a copy of the hospice discharge summary and the patient’s record, if requested. This standard was not met as evidenced by clinical record review, document review and interview. In 1 of 1 (100%) discharged clinical records reviewed the agency did not ensure the patient’s attending physician was forwarded a copy of the hospice discharge summary. Agency Policy #HC.45, Discharge from Hospice Care (revised date 05/2024) read in part, “A copy of the discharge summary is provided to the attending physician.” CLINICAL RECORD 8: The patient elected hospice care on 11/01/2024 with a hospice diagnosis of progressive supranuclear ophthalmoplegia. The patient was discharged on 12/20/2024 due to no longer terminally ill. The clinical record lacked documented evidence that the discharge summary was forwarded to the patient’s attending physician. The Administrator confirmed the findings in a joint record review and interview on 01/09/2025 and stated in part “I know we updated the physician, but it is not documented.” Additionally, the Vice President of Operations confirmed the findings and stated in part “Our documentation system does not generate a discharge summary. Our process is to send all the required components of the discharge summary and document it.””

    Rule L0683. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
