What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Hospice aide assignments and duties (42 CFR 418.76(g)(2)(ii))

    The inspectors’ words, 323 words

“It is required that a hospice aide provides services ordered by the Interdisciplinary Group and included in the plan of care. This standard was not met as evidenced by clinical record review, interview, and observation. In 1 of 1 (100%) clinical record reviewed, in which hospice aide services were accepted, revealed that the agency did not ensure the aide was providing services as ordered by the Interdisciplinary Group (IDG) and included in the plan of care (POC). Clinical Record 1: The plan of care orders, effective 02/01/2024, included a frequency of visits for the aide of three (3) per week. For the week of 02/01/2024, only one (1) visit, dated 02/02/2024, was documented as completed. There were no additional aide visits documented or documented evidence to support why two (2) visits were missed. The hospice aide care plan, effective on 02/01/2024, documented instructions for the aide to provide a complete bed bath every visit. The aide documented completed visits on 02/02/2024, 02/05/2024, 02/07/2024, 02/09/2024, 02/12/2024, 02/14/2024, and 02/16/2024; however, the aide’s clinical notes did not contain documented evidence to support that the bed bath was provided or documentation that the patient declined the bath. On 02/20/2024 a joint home visit (1) was made with the Medical Social Worker (Staff 3) and an interview was conducted with the patient on services provided. The patient stated the hospice aide did provide a bed bath on each visit. On 02/21/2024 a joint clinical record review and interviews were conducted with the Administrator and Director of Nursing, who confirmed that the clinical record did not contain documented evidence of a physician’s order for an aide visit frequency of one (1) visit per week for the week of 02/01/2024. Leadership also confirmed that the clinical record did not contain documented evidence to support that the aide provided a bed bath as instructed, for the identified visit dates, or documented evidence to support that the patient declined the bath.”

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

From the hospice

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
