What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Hospice aide assignments and duties (42 CFR 418.76(g)(4))
The inspectors’ words, 207 words
“It is required that hospice aides must report changes in the patient’s medical, nursing, rehabilitative, and/or social needs to a registered nurse as the changes relate to the plan of care and any quality assessment and improvement activities. This standard was not met as evidenced by clinical record review and interview. In 1 of 2 (50%) clinical records reviewed of patients who elected hospice aide services, the agency did not ensure the hospice aide reported changes related to the plan of care to the registered nurse. CLINICAL RECORD 8: The plan of care dated 06/28/2026 to 08/26/2026 included an order for hospice aide services twice a week. The clinical record review did not provide evidence that the hospice aide reported changes to the registered nurse. The hospice aide documented on the aide visit note dated 07/15/2026 that the Patient/caregiver declined the visit. The aide documented “No” on the visit note for the questions related to, was the case manager notified, and was the physician notified. During an interview with the Registered Nurse/Director of Nurses (Personnel Record 1) on 08/03/2026, she stated that the aides are verbally reporting it to the case manager but that they will provide education to the staff to document that information as well.”

Training (42 CFR 418.100(g)(3))
The inspectors’ words, 206 words
“It is required that the hospice assesses the skills and competencies of all individuals providing care, including volunteers providing services and, as necessary, provides in-service training and education programs where required. This standard was not met as evidenced by personnel record review, document review, and interview. In 2 of 9 (22%) personnel records reviewed revealed that the agency did not ensure to assess the skills and competencies of all individuals providing care, including volunteers providing services and, as necessary, provides in-service training and education programs where required. Document 1: The agency policy titled Administrative Policy Manual; Staff Competency HR.10 (Last reviewed 111524) stated in part “The Hospice will provide for competency assessments by qualified individuals on hire and annually to assess direct care staff’s basic levels for all staff providing care, including volunteers (if applicable), contracted staff, paraprofessionals and professionals.” Personnel Record 6: The personnel record review for the Licensed Vocational Nurse (LVN), with a hire date of 05/11/2020, did not provide evidence of an annual competency assessment for 2024 as required by agency policy. Personnel Record 7: The personnel record review for the Registered Nurse (RN) with a date of hire of 07/28/2025 did not include evidence of a competency at the time of hire.”
