What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 344 words
“Observed in Record Review. In 16 of 18 patient records reviewed, it was noted that the written plan of care was not individualized for each patient. For example, in record RRHV#1 8 of 9 goal’s time frames were in the past and had not been updated. Also, the wound care intervention had not been updated when the order was changed 3 months ago from unna boot dressing to wound cream #4 dressing. For example, in record RRHV#2 the aide’s POC target dates for all goals were 3 months in the past and not updated. Also, in RRHV#2 and RRHV#6 the MSW goals were Resolved with no current goals or interventions although the patient’s record included visit orders and visit notes from the MSW. For example in record RRHV#3 the MSW goals, in record RRHV#5 the Spiritual Counselor goals and in record RRNHV#1 the aide goals time frames were dated in the past. For example in record RRHV#5 the goals were not individualized; Judith will feel the sense that God is with her in her situation in spite of and Judith achieves feeling of spiritual comfort. In records RRNHV#1, #4, #5, #6, #7, #8, #9, #10 and #11 the goals time frames were dated in the past.
