What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Competency evaluation (42 CFR 418.76(c)(1))
The inspectors’ words, 341 words
“It is required that all hospice aide services must be provided by an individual who has successfully completed one (1) of the four (4) training and competency evaluations programs described below. A qualified hospice aide has successfully completed a competency evaluation program that: evaluates the aide’s competency in the following subject areas by observation of the aide’s performance with a patient or pseudo-patient: sponge, tub, and shower bath; [and] hair shampoo (sink, tub, and bed). The competency evaluation is performed by a registered nurse in consultation with other skilled professionals, as appropriate. This standard was not met as evidenced by clinical record review, interview, observation, and personnel record review. In 1 of 2 (50%) personnel records reviewed for Hospice Aides (HA), the hospice did not ensure that HA competency was evaluated for all assigned tasks.
