What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Content of plan of care (42 CFR 418.56(c))

    The inspectors’ words, 250 words

“It is required that each patient’s individualized written plan of care identifies planned interventions based on problems identified in the initial and updated comprehensive assessments. This standard was not met as evidenced by clinical record review, document review and interview. In 2 of 19 (11%) clinical records reviewed, the agency did not ensure that planned interventions based on problems identified in the initial and updated comprehensive assessments were included in the patients’ individualized written plans of care. CLINICAL RECORD 1: This patient elected the hospice benefit on 01/20/2025 with a terminal diagnosis of Alzheimer’s Disease. The Registered Nurse initial comprehensive assessment dated 01/20/2025 stated the patient was diagnosed with dysphagia in October 2024 and was at risk of aspiration. The plan of care dated 01/20/2025 through 10/21/2025 did not include interventions related to dysphagia. CLINICAL RECORD 8: This patient elected the hospice benefit on 06/06/2025 with a terminal diagnosis of COPD. The initial Registered Nurse initial comprehensive assessment dated 06/06/2025 stated the patient was admitted with Doxycycline 100 mg two times a day for the diagnosis of pneumonia. The drug profile was reviewed for the dates of 06/06/2025-12/11/2025 and included a Z-Pack for a respiratory infection initiated on 09/12/2025. The plan of care dated 06/06/2025 through 12/11/2025 did not include interventions related to the diagnosis of pneumonia or respiratory infection. During joint clinical record review and an interview on 12/11/2025, the Administrator stated there were no interventions in the plans of care to address the problems identified in the assessments.”

    Rule L0545. Standard-level.

  • Prevention (42 CFR 418.60(a))

    The inspectors’ words, 389 words

“It is required that the hospice follows accepted standards of practice to prevent the transmission of infections and communicable disease, including the use of standard precautions, such as hand hygiene, use of gloves, mask, eye protection, or face shield depending on anticipated exposure, safe handling of equipment in the patient’s care environment if it is likely to have been contaminated with body fluids, safe handling of soiled items in the patient’s care environment, other requirements of applicable state and federal law and regulation. This standard was not met as evidenced by clinical record review, document review, interview and observation. In 1 of 4 (25%) home visit observations, the agency did not ensure that the accepted standards of practice to prevent the transmission of infections and communicable disease, including hand hygiene and use of gloves, were observed by the staff. The Agency Procedure: IPC.11_A2 “Infection Control: Bag Technique and Hand Hygiene with Competency”, stated in part, “When to wash hands with soap and water...after using gloves.... When to use hand gel (sanitizer): before donning gloves and after removing gloves...moving between clean and contaminated body surfaces/sites...after contact with inanimate objects in the patient’s environment...before and after entering your bag.” HOME VISIT 2/CLINICAL RECORD 2: This patient elected the hospice benefit on 10/10/2025 with a diagnosis of senile degeneration of the brain. During an observed home visit conducted with the Hospice Aide (HA - Personnel Record 2) on 12/17/2025, the HA was observed to sanitize her hands and don gloves. She removed the patient’s wet brief and pad and used a washcloth to cleanse the perineal area and placed the soiled washcloth in the wash basin. The HA then used disposable wipes to cleanse the patient’s face and upper body. The HA reached for mouth swabs from the nightstand and cleansed the mouth without changing gloves or sanitizing hands. The HA then opened the drawer of the chest to retrieve new socks and grabbed the same washcloth from the basin and used the same water and wash cloth to cleanse the legs and feet that was used to cleanse the perineal area. The HA did not change gloves or sanitize hands during the bed bath. During an interview conducted on 12/17/2025, The Area Director of Quality Outcomes stated that it was surprising that this HA did not follow the infection control policy.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

From the hospice

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
