What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 284 words
“It is required that a patient clinical record containing past and current findings is maintained for each hospice patient. The record contains correct clinical information that is available to the patient’s attending physician and hospice staff. The record may be maintained electronically or on paper. (L670) Each patient’s record includes, at a minimum physician orders. This standard was not met as evidenced by clinical record review and interview. In 1 of 2 (50%) clinical records reviewed of patients with wounds, the hospice did not ensure the clinical record contained a physician’s order to discontinue wound care. Clinical Record 1: The patient elected the hospice benefit on 8/8/2023 with a diagnosis of Unspecified Protein Calorie Malnutrition. On admission, the patient had a documented wound on bilateral buttocks. On 8/30/2023, the physician ordered: “cleanse wound with cleanser. Apply Xeroform and cover with island dressing two times a week with skilled nurse visit. May change as needed for dislodgement and/or soilage.” The Licensed Practical Nurse (LPN) visit note dated 8/30/2023 did not include documentation that wound care was provided. The Registered Nurse (RN) visit note dated 9/1/2023 did not include documentation of wound care. The RN visit note dated 9/1/2023 documented that the wound was healing. The RN visit notes dated 9/5/2023 and 9/7/2023 did not include documentation that wound care was provided. The LPN visit note dated 9/12/2023 did not include documentation that wound care was provided. A clinical record review revealed no documentation that the wound was healed. There was no evidence of a physician’s order documenting the discontinuation of the wound care. During an interview, the Director of Clinical Services stated that the wound was healed, which was why wound care was not provided.”

- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
