What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Supervision of hospice aides (42 CFR 418.76(h)(2))

    The inspectors’ words, 235 words

“It is required that a registered nurse makes an annual on-site visit to the location where a patient is receiving care in order to observe and assess each hospice aide while he or she is performing care per the hospice’s policies and procedures. This standard was not met as evidenced by document review, interview, and personnel record review. Agency policy #7006, “Competency Assessment Policy” (dated 2019), stated in part, “A competency assessment will be completed at least 1 time per year... The agency will use a competency skills performance checklist developed specifically for each clinical job category...the clinical manager will evaluate the competence in performing and rendering care according to organization policies.” In 1 of 1 (100%) personnel record reviewed of a Hospice Aide who required an annual on-site visit, the agency did not ensure that the Registered Nurse (RN) made an annual on-site visit to observe and assess the Hospice Aide (HA) while the HA performed care. Personnel Record 8: The HA was hired on 06/08/2022. A personnel record review revealed an initial competency done upon hire. There was no documentation of an annual on-site visit by the RN in 2023 and 2024 to observe and assess the HA while performing care. During a joint review of the personnel files on 03/18/2025, the Regional Director confirmed there was no annual on-site visit documented with the RN to observe the HA for 2023 and 2024.”

    Rule L0632. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
