What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Governing body and administrator (42 CFR 418.100(b))
The inspectors’ words, 217 words
“Based on documents reviewed, it was determined the hospice failed to produce a policy and procedure that identifies another individual to assume administrator duties and responsibilities when the administrator is not available. The hospice also failed to provide documentation that identifies an individual, approved by the governing body, to assume administrator responsibilities when the administrator is not available. Findings: On 11/12/2024 at 1:40 PM, a list of documents to be reviewed was presented to the Hospice Director. Included on this list was a request for documentation with identity of, and governing body authorization for, the person who is authorized to act on behalf of the administrator during the administrator’s absence. On 11/13/2024, a second request was made to the Director for documentation of the person who is authorized to act on behalf of the administrator during the administrator’s absence. The documentation was not provided. On 11/18/2024, Director emailed Surveyor for clarification on this standard of the CoPs. The Director stated, “We are having a tough time locating this information.” The documentation was not provided. During an interview with the Director on 11/26/2024 at 2:45 PM, the above document was discussed. The Director acknowledged they did not have the document stating, “We have searched through the policies and procedures and could not find a document with this information.””

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
