What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content (42 CFR 418.104(a)(2))
The inspectors’ words, 125 words
“Upon patient record review, 1 of 4 (Patient #1) did not contain evidence that each record contained: A signed Election of Hospice Benefit Statement, in accordance with 418.24: Patient #1: SOC 09/01/2025. During Surveyor-attended home visit on 09/03/2025, the patient showed Surveyor his Medicare A/B card, and his AARP/United Medicare Supplement cards. During clinical record review, it was noted that the patient had not signed/dated an Election of Hospice Benefit Statement. The Alternate Administrator reported that the Hospice had decided to provide Pro Bono Hospice care/services to this patient. The admitting nurse had made a note on the bottom/end of the EOB that the content had been reviewed with the patient, but not signed, as the Hospice would not be billing the patient’s Medicare benefit.”

“Upon Inpatient Contract review, 1 of 4 (Silver/GIP/RIP) did not contain verbiage requiring that a copy of the patient’s Discharge Summary must be provided to the Hospice at the time of discharge from any inpatient care/services stay. Corrected on site.”
