What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Homebound HHA/Hospice Inform EP Officials (42 CFR 418.113(b)(2))
The inspectors’ words, 344 words
“It is required that the hospice implements emergency preparedness (EP) policies and procedures, based on the emergency plan, the risk assessment, and the communication plan, to include: the documented discussion of the plan provided to the patient/family and maintained by the hospice, This standard was not met as evidenced by clinical record review, document review, interview and observation. In 2 of 4 (50%) observed home visits it was revealed that the agency did not ensure that the patient/ family were provided the emergency plan as per agency policy. Agency policy, “Patient Emergency Classification-0007(3),” stated in part, “The emergency classification priority is documented on the patient’s emergency plan...The original is left in the patient’s home folder...The priority will be reviewed at each comprehensive assessment and updated as necessary based on the patient’s condition.” HOME VISIT 2 / CLINICAL RECORD 2: The patient elected the hospice benefit on 03/13/2026. During a home visit with the Registered Nurse (RN) (Personnel Record 3) on 08/11/2026, the patient’s home folder was reviewed and revealed that the patient’s emergency classification priority form was incomplete. The clinical record was reviewed, and it did not provide evidence of a completed emergency classification priority form, as required by agency policy. HOME VISIT 4 / CLINICAL RECORD 4: The patient elected the hospice benefit on 04/09/2026. During the home visit with the Chaplain/Bereavement Coordinator (Personnel Record 5) on 08/11/2026, the patient’s home folder was reviewed and revealed that the patient’s emergency classification priority form was incomplete. The clinical record was reviewed and revealed evidence of a completed emergency classification priority form but did not provide evidence that the patient/ caregiver received the updated form. During a joint record review and interview with the Director of Services RN (Personnel Record 8) on 08/12/2026, the Director of Services RN confirmed there was no documented evidence in the home binder of a completed Patient Emergency Classification Form for Home Visit #2 and Home Visit #4. The Director of Services RN stated, “They are not leaving the original in the home and only completing it in the EMR.””
