What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“Based on interviews and record reviews, the agency failed to:1. Document signs and symptoms of infection on a patient’s assessment (Patient #4); and, 2. Accurately document the services a patient received (Patient #7).This deficient practice was identified for 2 (Patient #4, Patient #7) of 20 (Patient #1, Patient #2, Patient #3, Patient #4, Patient #5, Patient #6, Patient #7, Patient #8, Patient #9, Patient #10, Patient #11, Patient #12, Patient #13, Patient #14, Patient #15, Patient #16, Patient #17, Patient #18, Patient #19, Patient #20) sampled patients reviewed for documentation requirements. Findings: On 11/05/2025 at 4:15PM, S1Administrator and S2Director of Nursing (DON) was presented with the below findings, and the surveyor requested that if they wanted to present any additional evidence to dispute the deficient practice, to do so by 11/06/2025. Review of the agency’s Clinical Record Review policy, revised on 10/2015, revealed, in part, clinical records were to be reviewed for completeness and accuracy. Patient #4Review of Patient #4’s physician order dated 09/04/2025 revealed Patient #4 was prescribed Keflex (antibiotic a medication used to treat infections) for a toe infection. Review of Patient #4’s record revealed no documented evidence of an assessment of Patient #4’s toe had been documented, nor any documented evidence of an assessment of Patient #4’s response to the infection. In an interview on 11/06/2025 at 2:43PM, S2DON indicated Patient #4’s record did not include the required documentation related to Patient #4’s toe infection assessment. Patient #7Review of Patient #7’s Plan of Care for the certification period of 07/29/2025 through 10/26/2025 revealed no documented evidence hospice aide services were ordered for Patient #7. Review of Patient #7’s record for the certification period of 07/29/2025 through 10/26/2025 revealed no documented evidence hospice aide services had been provided to Patient #7. Review of Patient #7’s skilled nursing notes dated 09/15/2025, 09/21/2025, and 09/25/2025 revealed the skilled nurse had completed hospice aide supervisory visits. In an interview on 11/06/2025 at 2:50PM. S2DON indicated the above mentioned hospice aide supervisory visits were inaccurate because Patient #7 had not received hospice aide services.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

From the hospice

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
