What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 162 words

“It is required that Hospice aides are assigned to a specific patient by a registered nurse who is a member of the Interdisciplinary Group. Written patient care instructions for a hospice aide are prepared by a RN who is responsible for the supervision of the hospice aide. This standard was not met as evidenced by clinical record review. In 1 of 10 (10%) clinical records reviewed of patients that received Hospice Aide (HA) services, the agency did not ensure that the HA received written instructions to meet the needs of the patient. Clinical Record 13: The patient elected hospice care on 09/26/2024 with a diagnosis of Alzheimer’s disease. The HA care plan dated 09/26/2024 assigned the HA the following tasks every visit: “shower, shampoo, shaving, assistance to the toilet, perineal care, dressing, and oral care.” The HA care plan was not updated when the patient’s condition changed and was determined to be imminent, and they started receiving bed baths on 10/08/2024. .”

  • Hospice aide assignments and duties (42 CFR 418.76(g)(2)(i))

    The inspectors’ words, 159 words

“It is required that a hospice aide provides services: ordered by the Interdisciplinary Group. This standard was not met as evidenced by clinical record review and interview. In 1 of 10 (%) clinical records reviewed, the agency did not ensure that the Hospice Aide (HA) provided services ordered by the interdisciplinary group. Clinical Record 2: The patient elected hospice care on 08/30/2022 with a diagnosis of cerebral infarction. The patient resided in an assisted living facility (ALF). The plans of care (POC) dated 01/22/2024 and 05/21/2024 ordered HA services two (2) times weekly. A clinical record review revealed no documentation of HA visits the week of 02/04/2024, one (1) HA visit the week of 02/11/2024, and one (1) HA visit the week of 06/16/2024. During a joint record review and interview on 10/22/2024, the Director of Operations confirmed that the Hospice Aide frequency was not as per the plans of care for the weeks of 02/04/2024, 02/11/2024, and 06/16/2024.”

    Rule L0626. Standard-level.

    The inspectors’ words, 257 words

“It is required that hospice aide provides services: included in the plan of care. This standard was not met as evidenced by clinical record review and interview. In 3 of 10 (30%) clinical records reviewed of patients that received Hospice Aide (HA) services, the agency did not ensure that the HA provided services included in the plan of care. Clinical Record 2: The patient elected hospice care on 08/30/2022 with a diagnosis of cerebral infarction. The patient resided in an assisted living facility (ALF). The HA care plan dated 08/30/2022 assigned the HA the task: “Shampoo is to be completed with every aide visit.” The HA visit note dated 07/12/2024 documented, “Shampoo not completed this visit.” Clinical Record 5: The patient elected hospice care on 09/24/2024 with a diagnosis of heart failure. The patient resided in an ALF. The HA care plan dated 09/24/2024 assigned the HA the task: “Shaving is to be completed with every visit.” A review of the HA visit notes revealed that “shaving” was documented as “not required” on every HA visit note. The HA care plan dated 09/18/2024 assigned the HA the task: “shower every visit.” The HA visit note dated 10/10/2024 documented a “full bed bath, not a shower.” During a joint clinical record review and interview on 10/23/2024, the Director of Operations agreed the hospice aides had not completed all the required tasks on the care plans.”

    Rule L0626. Standard-level.

What do families say?

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
