What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Names and Contact Information (42 CFR 418.113(c)(1))
The inspectors’ words, 215 words
“Based on a review of the agencys Emergency Plan, and staff interview it was determined the agency failed to develop and maintain an emergency preparedness communication plan that complies with federal, state, and local laws. The plan must include names and contact information for employees, entities providing services under arrangement, patient physicians, volunteers, and other hospice agencies. Failure to have a complete communication plan could result in harm to both patients and staff during an emergency. The following deficiencies were identified during the review of the agencys emergency plan: Incomplete Contact Information: The emergency plans communication section did not include names and contact information for all required parties, including: Employees Entities providing services under arrangement Patient physicians Volunteers Other hospice agencies Lack of Procedures: The plan did not outline procedures regarding when and how to contact volunteers and other hospice agencies during an emergency. The Hospice Manager was interviewed on 2/26/25 beginning at 9:45 AM, and the Emergency Preparedness plan was reviewed in her presence. During the interview, the Hospice Manager confirmed the absence of the required contact information, and the missing procedures related to the agencys communication plan. The agency failed to develop and maintain a comprehensive emergency preparedness communication plan that includes all necessary components as required by federal, state, and local laws”

EP Testing Requirements (42 CFR 418.113(d)(2))
The inspectors’ words, 260 words
“Based on a review of the agency’s Emergency Plan, and staff interview, it was determined that the agency failed to test its emergency preparedness plan annually. Failure to conduct regular testing of the emergency preparedness plan may result in insufficient knowledge of emergency procedures, potentially jeopardizing the safety of both patients and staff. The following details were noted during the review of the agencys emergency plan: Cybersecurity Exercise: The first documented exercise was titled a “;Cybersecurity Exercise,” conducted by the local hospital. However, there was no documentation indicating that any staff members from the hospice attended or participated in the exercise. Decontamination Mobil Unit Exercise: The second documented exercise involved testing the local hospitals Decontamination Mobil Unit in the event of an emergency. Similarly, there was no documentation confirming hospice staff participation in this exercise. Data Breach Incident: The third document provided was a one-page Word document detailing an incident in which the hospice manager experienced a data breach. There was no documentation to show that the agencys emergency plan was activated or deactivated, that the communication plan was utilized, that all staff were notified, or that an after-action report was completed. The Hospice Manager was interviewed on 2/26/25, beginning at 9:45 AM, at which time the Emergency Preparedness plan and associated exercises were reviewed in her presence. She confirmed that the hospice did not participate in the first and second documented exercises. Additionally, she acknowledged the lack of required documentation following the actual disaster the agency experienced. The agency failed to conduct annual testing of its emergency preparedness plan.”
