What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 278 words
“Based on observation, staff interviews, and review of agency policy and procedures, it was determined that the Certified Nursing Aide (CNA) AA failed to follow standard precautions to prevent the transmission of infectious by using dirty glove to apply barrier cream during bed bath for one patient (P) (P#16) of 19 sampled patients. The Findings include: Review of the policy titled, “;Hand Hygiene,” Policy: PCP-001, review date 6/20/23 revealed, “;Change gloves during patient care if moving from a contaminated body site to a clean body site.” During a home visit with P#16 on 3/19/2024 at 10:42 a.m., observed CNA AA perform complete bed bath care to P#16. During the bathing procedure, P#16 had a bowel movement. CNA AA wiped patients bottom with wet wipes, changed left handed glove and failed to change the right-hand glove. CNA AA used the left-hand glove to get barrier cream, squeezed it in right hand dirty glove and applied it to rub the bottom area of P#16. On 3/19/2024 at 10:58 a.m., an interview was conducted with CNA AA who confirmed that she did not change the right-hand dirty glove, and that she applied barrier cream to P#16s bottom with right hand dirty glove. CNA AA revealed that she changed gloves many times and she was confused that she did not change the right-hand dirty glove before she applied the barrier cream to P#16s bottom during the bathing procedure. During an interview on 3/19/2024 at 3:20 p.m., the Director of Nursing (DON), acknowledged that the CNA AA did not follow acceptable standards for infection control to prevent transmission of infectious diseases by using the dirty glove to apply barrier cream to P#16s bottom.”
