What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Hospice aide assignments and duties (42 CFR 418.76(g)(4))
The inspectors’ words, 278 words
“It is required that hospice aides must report changes in the patient’s medical, nursing, rehabilitative, and/or social needs to a registered nurse as the changes relate to the plan of care and any quality assessment and improvement activities. The hospice aide must also complete appropriate records of service, including the report of changes in the patient’s needs. This standard was not met as evidenced by clinical record review and interview. In 2 of 8 (25%) clinical records reviewed in which Hospice Aide services were provided, the agency did not ensure that the Hospice Aide reported changes in the patient’s needs to the Registered Nurse (RN). CLINICAL RECORD 1: The patient elected the hospice benefit on 06/07/2024. The plan of care (POC) dated 06/07/2024 assigned the hospice aide the relevant task: shampoo the patient’s hair one (1) time a week on Wednesday. The hospice aide visit notes dated 06/10/2024 and 06/17/2024 documented that the aide shampooed the patient’s hair on Mondays instead of on Wednesday as ordered. There was no evidence that the RN was notified of the change in the day that the task was provided. CLINICAL RECORD 3: The patient elected the hospice benefit on 04/16/2025. The POC dated 04/16/2025 assigned the hospice aide the relevant task: shave twice weekly. The hospice aide visit notes dated 07/28/2025, 07/29/2025, and 07/30/2025 documented the completion of shaving three (3) times for the week. There was no evidence that the RN was notified of the added tasks provided. During a joint review of clinical records and an interview on 08/04/2026, the Director of Clinical Operations confirmed the finding that the hospice aide did not inform the RN of the changes.”
