What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 319 words
“Based on clinical record review, review of agency documentation and interview with agency personnel, for one of thirty patients reviewed (Patient #23), the hospice agency failed to ensure the provision of medical social work services in accordance with physician’s orders and in response to the patient’s needs. The findings include: Patient #23 elected the Hospice Benefit on 10/22/25 and resided at home. The patient’s diagnoses included chronic obstructive pulmonary disease (COPD), depression, and anxiety. The physician orders for the certification period 6/19/26 through 8/17/26 directed nursing services one to two times weekly, hospice aide services one to two times weekly, medical social work services one to two times monthly, and volunteer services. Review of the Inter-Disciplinary Group (IDG) meeting note dated 7/8/26 identified documentation of a missed visit by the Medical Social Worker (MSW) on 6/30/26, with the MSW plan to visit Patient #23 prior to the next IDG meeting. A visit was planned for 7/9/26. Review of Patient #23’s clinical record identified no documentation for any MSW visit completion between 7/9/26 through 7/31/26. Interview and review of the Patient #23’s clinical record with the Interim Hospice Program Director (IHPD) on 7/31/26 at 12:43 PM did not identify the completion of any medical social work visits for the dates 7/9/26 through 7/31/26. The IHPD indicated the MSW visit on 7/9/26 was considered a missed visit, and there was no documentation completed by the MSW to indicate the reason for the missed visit, or notification to the medical provider of the missed MSW visit, or any documentation of the MSW’s attempts to reschedule the missed visit with the patient in order to meet the patient’s needs and physician orders. Review of the agency Documentation in the Clinical Record policy directed the record must reflect the coordination of service among all professional clinical staff involved in the patient’s care and with the physician(s), when a change in the visit frequency occurs.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
