The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 5 deficiencies in 1 month of the last 36. Last cited Nov 2023. Most serious: Standard-level. Every month

What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Notice of rights and responsibilities (42 CFR 418.52(a)(2))

“Observed in Patient Home. In 3 of 3 home visits conducted, the hospice did not provide the patient with written information about its policies on advance directives. The home folders at all three patient visits included the state of California information, but not the organization’s policy on advance directives. This was discussed with a Governing Body member, Administrator/DPCS and Office Manager.”

    Rule L0503. Standard-level.

  • Patient outcome measures (42 CFR 418.54(e)(2))

    The inspectors’ words, 93 words

“Observed in Patient Home. In 1 of 5 patient records reviewed, the hospice did not ensure that the patient record contained accurate information. In the record of RRHV#2 the medication list included Rosuvastatin 10 mg 12 tablets by mouth once daily. In that same record a registered nurse documented that the patient had left hemiplegia. During the home visit the surveyor observed that the patient was able to stand, walk, and move her left arm. There was no sign of hemiplegia. This was reviewed with a Governing Body member, Administrator/DPCS and Office Manager.”

    Rule L0535. Standard-level.

  • Content of plan of care (42 CFR 418.56(c))

    The inspectors’ words, 132 words

“Observed in Record Review. In 5 of 5 patient records reviewed, the organization did not develop plans of care that were based on patient’s goals and the time frames, settings, and services required to meet those goals. Most goals did not include specific measures to indicate progress and did not identify completion dates. For example: Patient, caregiver, family “will understand signs and symptoms of dying process prior to patient’s death; will achieve optimal grief reaction prior to patient’s death; will maintain optimal mobility during care within two weeks; pain will remain at a comfortable level during care within a two-week period of time; will demonstrate ability to manage bowel routine within two weeks; fall prevention will be maintained during care. This was reviewed with a Governing Body member, Administrator/DPCS and Office Manager.”

    Rule L0545. Standard-level.

  • Training (42 CFR 418.100(g)(3))

    The inspectors’ words, 103 words

“Observed in Record Review. In 5 of 5 patient records reviewed, the hospice did not adequately assess its patients. In every record reviewed the RN completing the initial comprehensive assessment also completed the spiritual assessment despite not having the education and training to do so. This was reviewed with a Governing Body member, Administrator/DPCS and Office Manager. Observed in Competency Session. In 1 of 9 HR files reviewed, the hospice did not assess competence initially as part of orientation. In HR#9 there was no competence assessment documented for the medical director. This was reviewed with a Governing Body member, Administrator/DPCS and Office Manager.”

    Rule L0663. Standard-level.

  • Content (42 CFR 418.104(a)(7))

“Observed in Record Review. In 1 of 5 patient records reviewed, the patient record did not include all medications the patient was using. For example, at RRHV#1 there was a prescription bottle labeled Amlodipine 10 mg one tablet by mouth daily that was filled three weeks prior to this visit. It was not included on the medication list in the patient’s home or their medical record. This was reviewed with a Governing Body member, Administrator/DPCS and Office Manager.”

    Rule L0678. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
