What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of comprehensive assessment (42 CFR 418.54(c)(6)(i))
The inspectors’ words, 251 words
“It is required that the documented comprehensive assessment includes a drug profile that contains the patient’s current: Prescription and over-the-counter (OTC) drugs; The drug profile is updated per the hospice policy and procedure, but, at minimum, at the time of each comprehensive assessment and/or when new medication is added or changes are made to existing medication. This standard was not met as evidenced by clinical record review and interview. In 1 of 3 (33%) home visits the agency did not ensure that the hospice medication profile was reconciled with each comprehensive assessment and updated with when new medication was added. CLINICAL RECORD 1: The patient elected hospice on 11/18/2025 with a diagnosis of Alzheimer’s disease. During the home visit (HV1) with the registered nurse (personnel record 6) on 01/09/2026, the facility medication profile printed on 01/09/2026 documented, on 12/15/2025, “Guaifenesin oral syrup 100mg/5ml, give 10ml by mouth every 8 hours as needed for cough.” The hospice medication profile printed 01/09/2026 did not include Guaifenesin. The registered nurse questioned the facility medication nurse who verified that the order was written on 12/15/2025 by the Nurse practitioner at the facility. During an interview on 01/09/2026, the director of patient care services reviewed the hospice medication profile and verified the medication was not entered onto the hospice medication profile as she had not been informed by the facility of the new medication order. She stated that this should have been discovered when reconciling medication records during the comprehensive assessment visit by the registered nurse.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
