What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Coordination of services (42 CFR 418.56(e)(1))
The inspectors’ words, 294 words
“Based on interview and record review, the agency failed to ensure the physician was notified of the missed visit in a timely manner for one of 14 sampled patients (Patient 4). This failure can result in inadequate communication among the care team, and the plan of care will not be adjusted according to the patient’s needs or desires. Findings: Per the Hospice Plan of Care (POC), dated 4/7/26 through 6/5/26, Patient 4 was admitted to the agency on 6/16/24 with diagnoses that included Parkinson’s Disease (a brain disorder that affects movement and other parts of the body). Per the POC, the Health Care Aide (HCA) was to visit Patient 4 twice a week. On 5/13/26, a review of Patient 4’s clinical record was conducted. The week of 4/12/26 through 4/18/26. Patient 4 was visited by the HCA once instead of twice that week, and the missed visit noted was created on 5/12/26. On 5/14/26 at 9:54 A.M., an interview and joint record review was conducted with the Director of Patient Care Services (DPCS). The DPCS stated that when the visit was missed, the HCA should have called the registered nurse case manager (RNCM), the RNCM should have created a missed visit note, and notified the physician for that week. The DPCS stated the physician was not notified timely of the missed visit. Per the agency’s policy and procedure, dated titled Missed Visits, “…if a visit is missed and not rescheduled, the clinician will: A. Notify the IDG [Interdisciplinary Group- the team who work together to meet the needs of the patient] and Director of Patient Care/Clinical Supervisor of the missed visit and reason for missed visit B. Document in the patient’s clinical record…and the IDG should evaluate the appropriateness of the plan of care…””

The inspectors’ words, 347 words
“Based on interviews and record review, the agency failed to ensure thorough documentation of a negative supervisory review for one of 14 sampled patients (Patient 1). This failure may result in a lack of accountability and ineffective follow-up on clinician performance concerns. Findings: Per the Hospice Plan of Care (POC), dated 4/20/26 through 6/18/26, Patient 1 was admitted to the agency on 10/22/25 with diagnoses that included hemiplegia (weakness on one side of the body). Per the POC, the Skilled Nurse was to visit Patient 1 once a week. On 5/11/26, a review of Patient 1’s clinical record showed that the Registered Nurse Case Manager (RNCM) conducted a supervisory review on 4/29/26. The RNCM documented the Licensed Vocational Nurse’s (LVN) name and marked “negative” under performance satisfaction but failed to provide any explanation or comments regarding the reason for the negative review. On 5/11/26, A review of the Grievance/Complaint Log was conducted, and there was no documentation about the RNCM reporting about Patient 1 or the representative’s negative concern or dissatisfaction toward the clinician. The RNCM was not available for an interview. On 5/14/26 at 9:54 A.M., an interview and joint record review were conducted with the Director of Patient Care Services (DPCS). The DPCS stated that during supervisory reviews, there are two options: negative and positive. The DPCS stated the RNCM should document in the comment section the reason for a negative review and notify them. The DPCS further stated she did not know why the LVN received a negative review. The DPCS stated it was important that a negative review was documented thoroughly and investigated by them to ensure the needs of the patient were taken care of and any concerns were addressed. Per the agency’s policy and procedure, dated 5/20, titled Complaint/Grievance Process, “To set forth guidelines for the resolution of patient concerns, dissatisfaction, or complaints and to protect patient and family rights…The hospice staff member receiving the complaint will discuss, verbally and in writing, the grievance with a supervisor within five (5) days of the alleged grievance. The supervisor will investigate the grievance…””

- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
