What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Timeframe for completion of assessment (42 CFR 418.54(b))
The inspectors’ words, 373 words
“Based on a review of clinical records and staff interviews, it was determined that the Medical Social Worker failed to complete and incorporate a psychosocial needs assessment into the initial comprehensive assessment within five days following the election of hospice services for two (#1 and #10) of fourteen (#1-#14) clinical records reviewed. This failed practice had the potential to affect all hospice admissions, as it could result in delayed or incomplete identification and management of psychosocial needs, placing patients and families at increased risk for emotional distress, ineffective coping, caregiver burden, inadequate support and resources, and unmet end-of-life needs. Findings follow: A review of clinical record #1, with a start-of-care (SOC) date 08/03/2026, revealed a patient diagnosed with Alzheimer’s Disease Unspecified.A Skilled Nursing Visit Note dated 08/03/2026 documented that the patient’s family agreed to an evaluation by the Medical Social Worker (MSW).The Interdisciplinary Group (IDG) Summary Report dated 08/04/2026 indicated that the MSW had not yet completed an evaluation and planned to schedule a visit with the patient’s family. The IDG Summary Report dated 08/18/2026 documented that the patient/caregiver was unavailable for the initial assessment, and the MSW planned to schedule the visit based on family availability. The IDG Summary Report dated 09/01/2026 noted that MSW services were not requested at that time and would be available as needed. No documentation of MSW visits, family contact, or completion of a psychosocial assessment was found in the clinical record. A review of clinical record #10, with a SOC date 08/27/2026, revealed a patient diagnosed with Chronic Obstructive Pulmonary Disease. The IDG Summary Report dated 09/01/2026 documented that the MSW was notified of the patient’s admission on 08/27/2026, but the patient/caregiver was unavailable for an assessment. The MSW planned to schedule a visit based on their availability. The IDG Summary Report dated 09/15/2026 indicated that the MSW had not seen the patient since the previous IDG meeting and planned to continue monthly and as‑needed visits. No documentation of MSW contact or completion of a psychosocial assessment was found in the clinical record. On 09/18/2026 at 1:30 PM, Agency Personnel #1 confirmed that the psychosocial assessment had not been completed by the MSW within the required five‑day timeframe following SOC, as mandated under the Medicare Hospice Conditions of Participation for the comprehensive assessment.”

- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
