What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Content of comprehensive assessment (42 CFR 418.54(c))

    The inspectors’ words, 284 words

“Based on clinical record review and staff interviews, it was determined that the Registered Nurse (RN) failed to complete a comprehensive wound assessment for 1 (#4) of 2 (#3 and #4) clinical records reviewed with documented wounds whose location of care was a skilled nursing facility. The absence of baseline wound characteristics and measurements necessary to support the development, implementation, and evaluation of the patient’s plan of care had the potential to impact all patients requiring wound care, as the RN was unable to independently evaluate and monitor the patients’ wound healing progress. A review of clinical record #4, with a Start of Care date of 06/29/26, revealed the patient was admitted to hospice with a diagnosis of Congestive Heart Failure. Review of the 06/29/2026 Skilled Nurse Hospice Admission Note showed the Registered Nurse (RN) documented the integumentary assessment as “06/29-2026 multiple venous and arterial wounds to bilateral feet, dressings intact.” Additional documentation stated: “During hospitalization, ulcerations on feet were evaluated, and an MRI of the left leg showed extensive osteomyelitis, avascular necrosis and gangrene were diagnosed.” However, there was no additional documented evidence that a comprehensive wound assessment was completed during the initial nursing comprehensive assessment. Specifically, the clinical record lacked documentation of wound characteristics, measurements, (length, width, depth), location-specific descriptions, drainage, wound bed appearance, peri wound condition, or staging/classification, as applicable. On 07/08/26 at 10:30 AM, Agency Personnel #1 confirmed the Hospice RN did not perform a comprehensive wound assessment on admission as required. Agency Personnel #1 acknowledged that, although the admission documentation identified multiple venous and arterial wounds to both feet and referenced osteomyelitis, avascular necrosis, and gangrene, the initial comprehensive nursing assessment did not include complete wound assessments or measurements.”

    Rule L0524. Standard-level.

  • Cost saving (42 CFR 418.78(d))

    The inspectors’ words, 176 words

“Based on a review of the administrative records and staff interviews, the hospice failed to maintain documentation demonstrating the use of volunteers, including volunteer hours and the associated cost savings, for review. This had the potential to affect the hospice’s ability to demonstrate compliance with the volunteer requirements at 42 CFR 418.78. Findings follow: During the survey, the surveyor requested documentation of volunteer hours and the volunteer cost savings report. Agency Personnel #1 and Agency Personnel #2 stated that the Volunteer Coordinator was currently on vacation for two weeks. They further reported that no other employee had access to the volunteer records or reporting system. As a result, the hospice was unable to produce documentation of volunteer hours or the volunteer cost savings report for surveyor review at the time of the survey. On 07/08/26 at 3:00 PM, Agency Personnel #1 and Agency Personnel #2 confirmed the hospice was unable to provide documentation demonstrating compliance with the volunteer documentation requirements, including verification of volunteer services provided and the associated cost savings, for the requested review period.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
