The short answer

Record last released by CMS on Oct 21, 2026.

The last 24 months 1 deficiency in 1 month of the last 36. Last cited Aug 2025. Most serious: Standard-level. Every month

Each figure’s source sits beside it, in its section below.

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Encoding And Transmitting Oasis

    The inspectors’ words, 254 words

“Encoding and transmitting OASIS CFR(s): 484.45(a) Standard: An HHA must encode and electronically transmit each completed OASIS assessment to the CMS system, regarding each beneficiary with respect to which information is required to be transmitted (as determined by the Secretary), within 30 days of completing the assessment of the beneficiary. This Standard is not met as evidenced by: Based on record review and interview, the Home Health Agency (HHA) failed to ensure the timely submission of OASIS (Outcome and Assessment Information Set) to IQIES (Internet Quality and Improvement and Evaluation System). The failure had the potential to affect all patients receiving services from the HHA who require evaluation and care. Findings include: Review of the agency’s policy titled, “Oasis Outcome and Information Set” reviewed on 06/09/25, revealed “The agency will collect, encode, and electronically transmit OASIS data as stated in the CMS Federal Conditions of Participation” directs staff to transmit OASIS information to the states at least every 30 days of the OASIS M0090 completion date. The agency’s OASIS submission data since Quarter 3, 2024 was reviewed for timely transmission to IQIES. The data revealed late transmissions over the state’s threshold of 10.0% per quarter for four of the four quarters. A total of 405 assessments were transmitted late. This resulted in an error code of -3330 (late submission). During an interview on 08/26/25 at 11:20 AM, the Director of Operations was asked about the delay in transmitting the OASIS information. The Director of Operations stated they were unaware of the late OASIS submissions.”

    Rule G0372. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
  • Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
