What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Assess B/P & Fluid Management Needs

    The inspectors’ words, 494 words

“PA-ASSESS B/P, FLUID MANAGEMENT NEEDS CFR(s): 494.80(a)(2) The patient’s comprehensive assessment must include, but is not limited to, the following: Blood pressure, and fluid management needs. This Standard is not met as evidenced by: 38766 Based on policy review, medical record review, and staff interviews, the facility staff failed to notify a nurse of a change in a patient ’ s blood pressure during hemodialysis treatments in one (1) of eight (8) sampled in-patient records review (Patient #8). Findings included: A review on 05/08/2025 of the facility’s policy titled “Intradialytic Hypotension” published 07/01/2024 revealed, “... Definition... Patient’s pre-tx [treatment]sitting SBP [systolic blood pressure] >/= [greater or equal to] 101 and </= [lesser or equal to] 109 mmHg [millimeters of mercury] AND has a 20 mmHg drop during the treatment... Policy... Staff, patient and/or care partner will report intradialytic hypotension episodes to the nurse in charge... Treating Hypotension on Hemodialysis... 3. Notify the nurse in charge...” An open medical record review on 05/08/2025 for Patient #8 revealed the patient was admitted to the facility on 03/09/2024 for ICHD treatment. The patient’s order summary revealed special attention indicated, “If the patient becomes symptomatic and/or PB drops below 90 systolic, call physician for orders.” A review of the patient’s treatment Flowsheet on 04/16/2025 revealed that hemodialysis treatment was provided to the patient from 0648 through 1017 for 3 hours and 29 minutes. The review revealed the patient’s pre-treatment blood pressure was 157/83. The review revealed a BP checked at 0650, indicating a reading of 139/82 with a comment by the PCT, “Resting comfortably; Treatment initiated without problem...” At 0744 (54 minutes later), the PCT documented a BP of 143/78; at 0801 (17 minutes later), a BP of 109/48; and at 0811 (10 minutes later), a BP of 109/48 with a comment by the PCT, “access visible, green AMP light, denies complaints.” At 0827 (16 minutes later), the PCT documented a BP of 111/36; at 0857 (30 minutes later), the BP was 109/63; at 0917 (20 minutes later), the BP was 124/55; at 0947 (30 minutes later) the BP was 121/66; at 1002 (15 minute later), the BP was 81/52; and at 1004 (2 minutes later) the BP was 79/46 and the PCT made a comment that “50 cc NS given due to pt hypotension.” The record review revealed no documentation of the nurse’s notification of the drop in the patient’s blood pressure during the treatment. An interview on 05/08/2025 at 1030 with the CM revealed that the expectation was for the PCT to have notified the nurse to assess and document the findings in the patient’s chart. An interview on 05/08/2025 at 1050 with PCT #3 revealed that if a patient experienced a drop in blood pressure during treatment, the staff was required to notify the nurse immediately. The PCT added that a Patient Care Technician (PCT) was permitted to administer normal saline but still had the responsibility to inform the nurse. The interview confirmed these findings.”
