What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Ic - Wear Gowns, Shields/Masks; Staff Not Eat/Drink In Tx Area
The inspectors’ words, 365 words
“IC-GOWNS, SHIELDS/MASKS-NO STAFF EAT/DRINK CFR(s): 494.30(a)(1)(i) Staff members should wear gowns, face shields, eye wear, or masks to protect themselves and prevent soiling of clothing when performing procedures during which spurting or spattering of blood might occur (e.g., during initiation and termination of dialysis, cleaning of dialyzers, and centrifugation of blood). Staff members should not eat, drink, or smoke in the dialysis treatment area or in the laboratory. This Standard is not met as evidenced by: 38766 Based on a review of facility policy, observations, and staff interviews, the facility staff failed to prevent cross-contamination by not ensuring that personal protective equipment (PPE) was worn outside of the treatment area for 2 of 2 observed staff leaving the treatment area (RN #1 and PCT #3). Findings included: A review on 10/08/2025 of the facility policy titled, “Personal Protective Equipment,” revised 11/06/2023, revealed “... Policy... Personal protective equipment, including a full-face shield or mask and protective eyewear with a full side shield, fluid-resistant gowns and gloves ...All personal protective equipment shall be removed prior to leaving the treatment area...” Observation on 10/08/2025 at 1127 in the patient treatment area revealed RN #1 and PCT #3 left the treatment area and entered the supply room while wearing their gowns used during patient care. A minute later, RN #1 and PCT #3 returned to the treatment area from the supply room with their gowns on. The observation revealed that the RN went to the nursing desk and the PCT went to station #2 to provide patient care. The observation indicated that RN #1 and PCT #3 failed to remove PPE (gown) before leaving the treatment area, as per facility policy. An interview on 10/08/2025 at 1130 with RN #1 revealed that the staff should not have left the treatment area wearing their gowns. The RN stated, “It just slipped my mind; I was not supposed to.” An interview on 10/08/2025 at 1140 with PCT #3 revealed that the staff went to the supply room to get some Saline. The PCT added that it was down on her after she returned to the treatment room, that she was supposed to remove the gown before exiting the treatment area. NC00233650”
