What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Pe - Building - Constructed/Maintained To Ensure Safety

    The inspectors’ words, 350 words

“PE-BUILDING-CONSTRUCT/MAINTAIN FOR SAFETY CFR(s): 494.60(a) The building in which dialysis services are furnished must be constructed and maintained to ensure the safety of the patients, the staff and the public. This Standard is not met as evidenced by: 17026 Based on observations and staff interview, the facility staff failed to maintain the facility environment in the storage room, biomedical technician’s office, and ceiling tiles to ensure the safety of patients and staff. This had the potential to effect all patients on dialysis and cause an increase in complications. The facility census was 53. Findings included: 1. Observation on 12/17/25 at 11:41 AM of the storage room showed a door used for deliveries that had an approximately five inch by one inch gap at the bottom of the door to the outside. Further observation showed the drywall adjacent to the door had an approximately three feet by one foot area and an approximately two feet by one foot area of drywall missing from the wall. 2 During an interview on 12/17/25 at 11:43 AM, the biomedical technician said the drywall had been in that condition for about one year after they had an issue with a water pipe. The biomedical technician said staff need to check with the facility landlord to ensure the areas get repaired. 3. Observation on 12/17/25 at 11:25 AM of the facility’s ceiling showed two ceiling tiles near the employee break room and four ceiling tiles in the storage room with brown and yellow stains. 4. During an interview on 12/17/25 at 1:02 PM, the director of operations said staff need to replace the stained ceiling tiles. 5. Observation on 12/17/25 at 11:32 AM of the biomedical technician’s office showed an approximately two feet by one foot area at the door entrance with brown and yellow areas on the floor and another approximately two feet by one foot area under the desk that had brown and yellow areas on the floor. 6. During an interview on 12/17/25 at 11:33 AM, the clinic manager said staff need to repair or replace the floor in the biomedical technician’s office.”

  • Appropriateness Of Dialysis Rx

    The inspectors’ words, 260 words

“PA-APPROPRIATENESS OF DIALYSIS RX CFR(s): 494.80(a)(2) The patient’s comprehensive assessment must include, but is not limited to, the following: (2) Evaluation of the appropriateness of the dialysis prescription, This Standard is not met as evidenced by: 17026 Based on record review and staff interview, the facility staff failed to follow the physician order for the blood flow rate during dialysis treatment for three (Patient # 1, # 2, and # 3) patients out of three clinical records reviewed. This had the potential to effect all patients on dialysis treatment and cause an increase in complications. The facility census was 64. Findings included: 1. Review of the treatment flow sheet dated 9/28/24 for Patient # 1 showed the patient’s physician directed staff to set the blood flow rate at 450 but staff set the blood flow rate at 400 for the patient’s dialysis treatment. 2. Review of the treatment flow sheet dated 8/20/24 for Patient # 2 showed the patient’s physician directed staff to set the blood flow rate at 400 but staff set the blood flow rate at 300 for the patient’s dialysis treatment. 3. Review of the treatment flow sheet dated 8/20/24 for Patient # 3 showed the patient’s physician directed staff to set the blood flow rate at 450 but staff set the blood flow rate at 400 for the patient’s dialysis treatment. 4. During an interview on 9/30/24 at 12:40 PM, the interim clinical manager said staff should follow the physician order for the blood flow rate or document the reason for not following the physician order.”
