What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Ic - Sanitary Environment

    The inspectors’ words, 423 words

“IC-SANITARY ENVIRONMENT CFR(s): 494.30 The dialysis facility must provide and monitor a sanitary environment to minimize the transmission of infectious agents within and between the unit and any adjacent hospital or other public areas. This Standard is not met as evidenced by: 36473 Based on observation, interview and policy review, the facility failed to ensure staff provided and maintained a clean environment free from potential contaminants when the staff failed to: - Maintain the pipes under the patient sink (side 1) free of rust (a reddish or yellowish-brown flaky coating formed on iron or steel that had the potential to harbor dangerous bacteria). - Maintain the left faucet handle for the back sink (dirty sink) free of rust. -Maintain one rolling cart near the nurse’s station free of rusted and chipped paint. - Maintain the nurse’s station countertop free of old tape (attracts bacteria). - Maintain the oxygen tank holder free of rust. - Maintain Station 9’s computer cart free of a personal cellular phone (harbors bacteria). These failed practices had the potential for increased risk of cross-contamination and transmission of pathogens to all patients and staff. The in-center hemodialysis (the process of purifying the blood of persons whose kidneys were not working) patient census was 95. Findings included: 1.Observation on 4/21/25 at 2:30 PM, showed on unit side 1, one patient sink with rusted pipes going into the wall and one back sink (dirty) with one left faucet rusted. 2.Observation on 4/23/25 at 11:30 AM, at the nurse’s station, showed the countertops with multiple pieces of unclean old tape (attracts bacteria). 3.Observation on 4/23/25 at 11:35 AM, in the common area near the nurse’s station, an unclean rolling cart and a rusted oxygen tank holder. 4.Observation on 4/23/25 at 11:45 AM, at Station #9, on the computer cart, showed Staff B’s (CCHT, Certified Clinical Hemodialysis Technician) personal cellular phone (harbors bacteria). 5.During an interview on 4/23/25 at 1:30 PM, Staff A, Facility Administrator, stated staff were to maintain the unclean areas and not have their cellular phones at the patient stations. 6.Review of the facility’s policy titled, “Infection Control for Dialysis Facilities,” last reviewed on 04/2023, showed the direction for staff to minimize the spread of infections or bloodborne pathogens in the dialysis facility environment and to follow the Centers for Disease Control and Prevention (CDC). 7.Review of the CDC recommendations, titled, “Core Infection Prevention and Control Practices for Safe Healthcare Delivery”, dated 11/29/22, showed the required insurance of accountability for the success of infection prevention activities in a health care facility.”
