What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Pe - Building - Constructed/Maintained To Ensure Safety

    The inspectors’ words, 421 words

“PE-BUILDING-CONSTRUCT/MAINTAIN FOR SAFETY CFR(s): 494.60(a) The building in which dialysis services are furnished must be constructed and maintained to ensure the safety of the patients, the staff and the public. This Standard is not met as evidenced by: 22960 Based on observation and interview, the facility failed to provide backflow protection for the drain lines of three of three portable Reverse Osmosis (RO) machines at Skilled Nursing Facility (SNF) B, and for six of six hemodialysis machines and three of three portable RO machines at Skilled Nursing Facility (SNF) C, resulting in the potential contamination of the hemodialysis machines and RO machines from backflow of contaminants. The deficient practice had the potential to affect all patients receiving dialysis treatment in the facilities. Findings include: On 8/7/24 at 10:00 AM, during an observational tour of SNF C, the drain lines from the hemodialysis machines and the portable RO machines were observed without an air gap to prevent backflow of gas, liquid, or solid contaminants. On 8/7/24 at 10:15 AM during an interview, CCHT “K” was unaware of the lack of air gap for the hemodialysis machine drain lines and the portable RO drain line. According to the 2021 Michigan Plumbing Code, incorporating the 2021 edition of the International Plumbing Code, Section 802 Indirect Waste. “802.1 Protection: Devices, appurtenances, appliances and apparatus intended to serve some special function, such as sterilization, humidification, distillation, processing, cooling, or storage of ice or foods, and that discharge to the drainage system, shall be provided with protection against backflow, flooding, fouling contamination and stoppage of the drain.” 39083 On 8/6/24 at 12:13 PM, the drain lines of the reverse osmosis (RO) units, located in the treatment area of SNF B, were observed to not have an air gap (a physical separation in the drain line that prevents potential backflow of sewage into RO units). The drain lines of the RO units were observed to be snaked down into the pvc drain pipe that is connected to the facility sanitary sewage drainage system. During an interview on 8/7/24 at approximately 12:15 PM, Biomed “M” stated that three RO machines are in operation at the SNF B dialysis den. According to the RO unit manufacturer’s “Operation and Maintenance Manual,” it notes, “... 4.3 INITIAL STARTUP... 5. The REJECT WATER TO DRAIN hose coming out of the MRO1 system is for reject water. The water from this hose will go down the sink. Leave at least a 2” air gap between the hose and drain to prevent contamination of siphoning.””

    Rule V0402. Standard-level.
