What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Ic - Handling Infectious Waste

    The inspectors’ words, 341 words

“IC-HANDLING INFECTIOUS WASTE CFR(s): 494.30(a)(4)(i) [The facility must demonstrate that it follows standard infection control precautions by implementing-] (4) And maintaining procedures, in accordance with applicable State and local laws and accepted public health procedures, for the- (i) Handling, storage and disposal of potentially infectious waste; This Standard is not met as evidenced by: 27408 Based on observation and interview, the facility failed to ensure biohazard waste was disposed of in a safe and appropriate manner, resulting in the potential for the spread of infection amongst the 11 patients receiving dialysis at the time of observation. Findings include: On 04/14/25 at 1020, during an initial tour of the treatment floor, three large biohazard bins were observed with no lids on. Each bin was lined with a red bag labeled “biohazard waste”, which was used for contaminated hemodialysis blood tubing. The biohazard bins were observed across from stations #3, #12 and #20 (in the isolation room). At the time of observation, Registered Nurse (RN), Staff N, was queried as to what the bins were used for, and if they should be uncovered once waste products were placed in the bin. Staff N stated, “those bins are for mostly bloody tubing. They should have the lids on.” On 04/14/25 at 1100, the Facility Administrator, Staff A, confirmed the findings. She stated that the lids were in the back and that they should be in use. Staff A went to the back storage room, returned with 3 red plastic biohazard bin lids, and placed the lids on the biohazard material bins, creating a tight fitting lid. On 04/16/25 at 1004, during review of policy #4-03-07, titled “Segregation of Medical Waste” it was noted “#3. When using option c (A labeled plastic container with no foot mechanism to open the lid), the following infection control practices need to be followed: * Lids should be tight fitting * Gloves should always be worn when removing the lid * Lid should be put back on the container after medical waste is disposed of and contaminated gloves removed”

    Rule V0121. Standard-level. From a complaint.
