What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Idt Develops Plan Of Care

    The inspectors’ words, 224 words

“POC-IDT DEVELOPS PLAN OF CARE CFR(s): 494.90(a) The interdisciplinary team must develop a plan of care for each patient. This Standard is not met as evidenced by: 15195 Based on record review and interview, the facility failed to have all members of the interdisciplinary team (IDT) involved in the plan of care for 1 (#3) of 5 patients reviewed, resulting in the potential for less than optimal outcomes. Findings include: Record review revealed that patient #3 was a 55-year-old male patient with diagnoses of hypertension, prostate cancer, end stage renal disease, and other comorbidities. He began peritoneal dialysis on 2/22/22 and developed peritonitis (inflammation/infection of the membrane lining the abdominal wall) in November 2023. On 11/15/23 the patient was made unstable but not all members of the IDT participated in the patient’s plan of care (POC). It was noted that the dietitian and social worker did not participate in the POC. This was verified by the Facility Administrator on 4/24/24 at approximately 1500. The facility policy titled “Interdisciplinary Team (IDT) Patient Assessment and Plan of Care, #5-15-02, revised 7/2023” documented, “The facility’s interdisciplinary team (IDT) consists of, at a minimum, the patient or the patient’s personal representative, a registered nurse, a physician or Non-Physician Practitioner (NPP), if allowed by state ESRD licensure regulations, treating the patient for ESRD, a social worker and a dietitian.””
