What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

These performance components must influence or relate to the desired outcomes or be the outcomes themselves. This Standard is not met as evidenced by: 27125 Based on document review and interview it was determined that the Facility failed to ensure that the quality program included trending and analysis of data collected for mortality to improve outcomes. Findings include: 1. The Facility’s policy titled, “Quality Assessment & Performance Improvement (QAPI) (effective 1/2021) was reviewed on 4/3/2024 and included, “Reporting tools will be used to: ... document ongoing measurement, analysis, and tracking of aggregate quality indicators. ... Allow for identification, prevention, and reduction of ... mortality and morbidity.” 2. The Quality data from 01/2023 - 03/2024 was reviewed for mortality on 4/3/2024. The data included that the facility had 4 deaths during 2023. Three deaths included the cause as “expired” and 1 death included a cause of sepsis. The individual deaths were never trended for cause or an analysis of the deaths conducted. 3. During and interview on 4/3/2024 at approximately 1:30 PM, the Clinical Manager (E#2) stated that each member of the team is assigned an outcome to follow. When a trend is identified, that team member reports it to quality. E#2 stated, “A cause of death should be listed so that an evaluation of any trends can be conducted. The information needs to be discussed in the quality meeting so the team can evaluate all the data together.””
