What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Manage Volume Status

    The inspectors’ words, 476 words

“POC-MANAGE VOLUME STATUS CFR(s): 494.90(a)(1) The plan of care must address, but not be limited to, the following: (1) Dose of dialysis. The interdisciplinary team must provide the necessary care and services to manage the patient’s volume status; This Standard is not met as evidenced by: 39943 Based on record review, and interview, the facility failed to ensure a patient had reached their target weight/estimated dry weight (EDW), during treatment, failed to implement a corrective plan when the EDW was not achieved, and failed to document the reason for changing the Blood Flow Rate (BFR) for 1 of 6 sampled patients, (#1). Findings: On 8/18/25, while completing a review of patient #1’s medical record, it was noted the physician prescribed EDW was 106 kilograms (kg) for 8/04/25 to present and 104 kg for 7/23/25 to 8/02/25. The patient was prescribed three treatments a week and the prescribed treatment time was three hours and forty-five minutes. Review of the treatment flow sheets revealed patient #1 did not achieve the prescribed EDW on 9 of 13 treatments. Patient #1 did not achieve the prescribed EDW goal on the following dates: 8/18/25-post weight 107.8 kg, 8/12/25-post weight 108.4 kg, 8/09/25-post weight 109.2 kg, 8/02/25 -post weight 106.7 kg, 7/30/25-post weight 106.1 kg, 7/28/25-post weight 105.8 kg, 7/26/25-post weight 106.2kg, 7/25/25-post weight 107.3 kg, and 7/23/25-post weight 11 kg. Review of resident #4’s Dietician and Social Work notes revealed no evidence of any discussion regarding the patient not meeting his EDW. Further record review for patient #1 reflected no evidence the patient was offered extra treatment time or an extra treatment day to meet his EDW. There was no documentation the physician was notified of the occasions the patient did not reach his EDW after each treatment. Review of the patient #1’s treatment orders revealed an order for the BFR to be 350 milliliters (ml) per minute. Review of treatment sheets revealed on 7/30/25 between 4:00 PM and 5:00 PM, the BFR was decreased to 300 ml per minute, and between 5:30 PM and 6:00 PM, the BFR was 285 ml per minute. On 8/19/25 at approximately 5:00 PM, the Administrator acknowledged the patient should have been offered an extra treatment or extra time during his treatment when the dry weight was not reached. Review of the Pre-Intra-Post Treatment Data Collection, Monitoring and Nursing Assessment Policy dated September 2007 and revised April 2024 revealed if the dialysis prescription was not met (including dialysis flow rate or change to/inability to obtain prescribed blood flow rate) the reason will be documented and the licensed nurse informed. The policy indicated members of the patient care team should report ANY changes in patient conditions or concerns of patient well-being immediately to the licensed nurse at any time. The document described the condition, if patient is above or below 1 kg from the target weight.”
