What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 346 words

“PA-FREQUENCY-INITIAL-30 DAYS/13 TX CFR(s): 494.80(b)(1) An initial comprehensive assessment must be conducted on all new patients (that is, all admissions to a dialysis facility), within the latter of 30 calendar days or 13 hemodialysis sessions beginning with the first dialysis session. This Standard is not met as evidenced by: 29722 Based on patient medical record reviews, staff interview and review of the facility’s policy and procedure, the facility failed to ensure the completion of a comprehensive assessent for 1 of 10 patients sampled with a change in modality. (Patient #1) The findings include: On 07/16/2024 a medical record review conducted for Patient #1, revealed Patient #1 was admitted on 06/03/2024, after transferring from another facility where the patient had been followed for peritoneal dialysis. Patient #1 was admitted to the facility for in-center hemodialysis treatment, a new treatment modality. The patient’s medical record failed to include a comprehensive assessment. On 07/16/2024 at approximately 3:15 PM, the comprehensive assessment for Patient #1 was requested from the Clinic Manager. On 07/17/2024 at approximately 10:00 AM, the Clinic Manager presented Patient #1’s comprehensive assessment / Plan of Care that she reviewed with the patient this morning. The Clinic Manager stated somehow the system didn’t flag him for a care plan and it was missed. The patient had a previous plan of care from February so the system did not flag him needing one. She confirmed that patient’s with a change in treatment modaility require a comprehensive care plan review within 30 days or 13 treatments. A review of the facility’s policy and procedure entitled “Comprehensive Interdisciplinary Assessment and Plan of Care,” Reference 45283, published on 07/03/2023, indicates on page 2 of 10, “An initital comprehensive interdisciplinary assessment (ICA) and plan of care (POC) must be conducted on all patients new to dialysis within the later of 30 calendar days or 13 outpatient hemodialysis sessions beginning with the first outpatient dialysis session. Patients changing modality are classified as new patients and must have comprehensive interdisciplinary assessments and plans of care completed with the same frequency as patients new to dialysis.””
