“Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member and is considered part of the staff member’s record. In the record, there was no documentation that Staff #5 who has been working for more than 90 days, completed orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities.”

The center is required to have employees undergo a background check every five years.

“The most recent national criminal history background check for staff #3 was overdue by 10 months.”

Annual approval from the health department shall be provided for meeting requirements for: 1. Water supply; 2. Sewage disposal system; 3. Food service, if applicable.

“The center failed to request the health department complete an environmental sanitation inspection annually. The last environmental sanitation inspection from the health department was dated over 17 months ago.”

Based on a documentation review the facility failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

“Evidence: According to center documentation, child, C6, age 4, ?On Wednesday, April 17, 2024 a student, was found outside the classroom door unattended at 12:30. She was left unattended for approximately 10-15 minutes?. The center acknowledged the incident and completed at the time a review of the incident and supervision policy.”

“Based on review of the center’s written record of children serious or minor injuries the center failed to document required information. Evidence: The written records for serious or minor injuries for C8 (06/20/2024), C9 (06/06/2024), C10 (06/06/2024) and C11 (06/06/2024) did not document the time when parents were notified of the injuries and how the parents were notified of the injury. The elements on the written record were not completed.”

Based on review of written records of child injuries, the center failed to inform the department’s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
