What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Jan 13, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernThe Office of Child Care Licensing received a complaint on November 19, 2025, regarding a lack of supervision, meals and snacks, and children having access to hazards items. An investigation was conducted on December 1, 2025. There is no evidence to substantiate this complaint because Provider#1 stated the button battery that the Reporter stated was found in Child #1 s stool did not come from her child care, since the toys in her child care only use double or triple A batteries, and the covers must be secured with a screwdriver. Provider #1 also stated Child #1 was picked up around 4:00 pm and was in the care of the parent for two hours before she was told of the incident around 6:00 pm. During the visit, LS #1 inspected several toys that required batteries. All toys observed appeared to be secured and required a screwdriver to remove the cover and expose the batteries. Also, Provider #1 stated that Parent #1 provides the meals and snacks for the children, and she only feeds them what the parent provides for them that day. During the investigation, concerns noted were children present were resting on nap mats without a bottom cover, and the provider did not have the week s menu posted and was unable to provide a menu for that week.
May 27, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint regarding a lack of supervision, not providing positive behavioral support, meals and snacks, and not following the expulsion policy. An investigation was conducted on May 5, 2025. There is no evidence to substantiate this complaint because Provider #1 stated she followed her policy when terminating child care. LS #1 reviewed Provider #1’s parent handbook and it states, “Your child care will be terminated in the following situations: you or a family member becomes a safety threat by becoming hostile, and we cannot resolve the situation; or you fail to pay for child care, as stated in my contact.” Provider #1 stated that she or the substitute supervises the children and that the children are fed nutritious meals
Mar 27, 2025OCCL Standards Complaint: No Evidence to SubstantiateOn February 13, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, being disrespect in front of children, and not following the expulsion policy. An investigation was conducted on February 25, 2025. There is no evidence to substantiate this complaint because Provider #1 denied yelling and stomping in front of children or leaving them alone in the child care area and stated Parent #1 kept going back and forth about payment reimbursement due to her closing due to snow. Provider #1 stated she followed her policy and terminated child care. LS #1 reviewed Provider #1’s parent handbook which stated, “Your child care will be terminated in the following situations: you or a family member becomes a safety threat by becoming hostile, and we cannot resolve the situation; or you fail to pay for child care, as stated in my contact.”
Mar 18, 2025OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint regarding not following the child health observation procedures. An investigation was conducted on February 10, 2025. This complaint is substantiated for not following the child health observation procedures because Provider #1 stated she observed the scratch over Child #1’s eye upon arrival at the family child care home but did not document or inform the parent. Provider #1 stated she assumed Child #1’s mother was aware of the scratch since Child #1 had long fingernails. Provider #1 stated she also observed Child #1 s puffy eyes upon arrival.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
