What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Sep 16, 2025OCCL Standards Complaint: No Evidence to SubstantiateOn August 18, 2025, the OCCL received a complaint regarding having a person present at the center without a comprehensive background check. An investigation was conducted on August 22, 2025. There is no evidence to substantiate this complaint because Administrator #1 stated after the previous complaint, Person #1 was no longer sleeping at the center; however, Person #1 is fingerprinted and is determined eligible and will help clean the center.
Aug 5, 2025OCCL Standards Complaint: SubstantiatedOn July 7, 2025, the OCCL received a complaint regarding illegal drugs being in the center, not keeping the center in a hazard-free and safe condition, keeping the grounds in a clean and safe condition, and not completing comprehensive background checks. An investigation was conducted on July 9, 2025. This complaint is substantiated because multiple staff members reported finding alcohol in the freezer, finding a blunt on the playground at the center, finding a vape pen in a classroom, finding a gun box with cut up straws inside in the center, and finding a folded dollar bill and a straw both with white residue in the bathroom used by the children. Staff also stated Person #1 was present at the center during and after business hours and Person #1 did not have a comprehensive background check completed.
Sep 16, 2024OCCL Standards Complaint: SubstantiatedOn July 29, 2024, the Office of Child Care Licensing received a complaint regarding lack of supervision, caring for toddlers and preschoolers in the same space, using a wading pool, having adults visit the center for personal reasons, serving juice to a child who cannot have it, and not having permission to walk to a local playground. An investigation was conducted on August 5, 2024. This complaint is substantiated for lack of supervision because upon arrival, a staff member left two children alone in a classroom to open the door, and the owner confirmed the allegation of Staff #2 being in the kitchen leaving the toddlers and preschoolers combined and alone with no supervision, but stated she was not aware the children were alone at the time. This complaint is substantiated for caring for toddlers and preschoolers in the same space because the owner confirmed the ages are sometimes mixed. This complaint is substantiated for using a wading pool, serving juice to a child who cannot have it, and walking to a local park without parent permission because the owner confirmed the allegations. There is no evidence to substantiate an adult visited for personal reasons because the owner stated the adult visitor was only there to assemble the slide and wading pool, and then left the premises.
Feb 8, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing received a complaint on December 21, 2023, regarding not keeping all areas, furnishing, toilets and equipment of the center in a clean, hazard-free, and safe condition. An investigation was conducted on February 5, 2023. There is no evidence to substantiate the allegations because the specialist observed everything in working condition and the owner submitted proof of the repairs.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
