The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.

What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| May 7, 2025 | OCCL Standards Complaint: Substantiated | The Office of Child Care Licensing (OCCL) cannot legally disclose if a complaint was also received or investigated by any other agency. OCCL investigations of regulatory non-compliance begin when clearance from other agencies, when applicable, have been received. The Office of Child Care Licensing (OCCL) received four complaints regarding improper discipline. An investigation began on April 2, 2025, and concluded on April 16, 2025. This complaint is substantiated because Staff #2 and Staff #4 received disciplinary actions for being rough with children in their care. Additionally, video footage reviewed showed Staff #2 roughly putting a child back on his nap mat, and Staff #4 was observed pushing a child roughly away from her and placing another child roughly in a seat. During the investigation, other concerns were the center thermostat read 65 degrees, a staff member was present at the center and did not have a comprehensive background check and eligibility determined, and a child was made to rest and not given a quiet activity after 30 minutes of not sleeping. |
| Mar 18, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing (OCCL) received a complaint regarding having an unsafe indoor space. An investigation was conducted on February 13, 2025. There is no evidence to substantiate this complaint because the licensing specialist did not observe or smell any fire residue during the visit. Additionally, Administrator #1 stated the center did not have a fire, but burnt food in the oven triggered the fire alarm. The fire department came to inspect the center and gave the center permission to return to the center. |
| May 20, 2024 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | The Office of Child Care Licensing received a complaint on March 28, 2024, regarding improper discipline and not providing truthful information. An investigation was conducted on April 4, 2024. There is no evidence to substantiate this complaint for improper discipline because Staff #3 stated she has never observed Staff #2 hit any children. Additionally, LS Donovan reviewed video footage, and it did not appear that Staff #2 hit any child. There is no evidence to substantiate not providing truthful information regarding the child getting bitten because the center was unable to produce the injury report, but Staff #2 stated Child #1 was bitten close to the end of the day. Additional concerns noted were the Reporter and Staff #2 stated Child #1 was bitten, but the center could not provide the injury report and a separate injury report reflected the parent was not notified Child #1 sustained a bruise to her face. |
| May 6, 2024 | OCCL Standards Complaint: Substantiated | The Office of Child Care Licensing received a complaint on April 4, 2023, regarding a staff member present without a comprehensive background check. An investigation was conducted on April 4, 2024. This complaint is substantiated because Staff #2 was present in the center without a comprehensive background check and was alone with children. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
