What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

DateWhatDetail
Dec 4, 2025OCCL Standards Complaint: SubstantiatedOn October 16, 2025, the Office of Child Care Licensing received a complaint regarding lack of supervision and accident/injury reporting procedures. An investigation was conducted on October 22, 2025. This complaint is substantiated for lack of supervision because although staff denied the child was injured while in care, during the visit, a group of children was left alone in a classroom while the staff answered the door. There is no evidence to substantiate the allegation of not following accident/injury reporting procedures because two classroom staff stated Child #1 had no injuries while in care. In addition, Child #1 was signed out at 5:05 p.m. and according to staff and Brightwheel documentation, the parent did not communicate Child #1 had an injury until 13 minutes after Child #1 left the facility. Additional concerns noted during the visit included improper staff-to-child ratio and insufficient staff onsite.
Jul 29, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn July 1, 2023, the Office of Child Care Licensing received a complaint about not being truthful and not following the facility s termination policy. An investigation was conducted on July 15, 2024. There is no evidence to substantiate the allegation of not being truthful because Brightwheel and what the staff stated was the same information. There is no evidence to substantiate the allegation of not following the termination policy because the parent handbook states if a parent is verbally abusive to a teacher or administrator, it will result in termination of care and termination is at the discretion of the administrator.
Jul 1, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn June 25, 2024, the Office of Child Care Licensing received a complaint regarding infant feeding. An investigation was conducted on June 26, 2024. There is no evidence to substantiate the complaint because the staff stated they discard formula after the one hour feeding time has been exceeded. The licensing specialist observed bottles in the classroom were clean, properly labeled, documented start times, and no cans of formula or breast milk were expired
Dec 1, 2023OCCL Standards Complaint: No Evidence to SubstantiateOn November 15, 2023, the Office of Child Care Licensing received a complaint regarding not following the center s termination policy and discrimination. An investigation was conducted on November 28, 2023. There is no evidence to substantiate the complaint because the center s parent handbook states, services may be suspended or terminated at any time for any reason. Also, staff stated the child s enrollment was terminated due to Parent #1 was not providing meal items as requested, Child #1 disturbing nap time, Child #1 attempting to hurt other children unprovoked, and Child #1biting Staff #1.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
