The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Sep 30, 2025 | OCCL Standards Complaint: Substantiated | OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. Between July 30, 2025, and August 4, 2025, the Office of Child Care Licensing received three separate complaints regarding failing to use appropriate behavior supports. An investigation began on August 8, 2025, and was concluded on September 15, 2025. After reviewing video footage and conducting interviews, this complaint is substantiated for failing to use appropriate behavior supports. All staff members interviewed stated that they did not witness Staff #1 strike Child #1. The video footage did not provide any indication that Child #1 was hit by Staff #1. However, video footage reviewed by LS #1 documented a harsh tone was used by Staff #1 when addressing Child #1. The video footage captures Child #1 approaching Staff #1 to show a drawing. In response, Staff #1 dismissed Child #1 using a harsh tone, saying, Go sit down! Administrator #1 stated they wrote up Staff #1 for the tone they used when speaking to Child #1. Another staff member stated Staff #1 can use a harsh tone toward the children. |
| Feb 9, 2024 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | On December 12, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding diapering procedures and insufficient staff. Based on interviews, documentation, and observations at the visit there is no evidence to substantiate this complaint for insufficient staff because it was observed that the center had sufficient qualified staff to supervise children and conduct other functions of the facility. Based on interviews, and observations at the visit there is no evidence to substantiate this complaint for incorrect diapering procedures. The diapering observed during the visit was in compliance with regulations and staff stated diapers are checked or changed at least every hour. A concern noted during the investigation was Staff #3 had been inactivated at the center on August 19, 2022, but was documented to be working at the center and did not have current proof of eligibility on file. An updated criminal background check and eligibility determination is required before she may work at the facility. |
| Nov 17, 2023 | OCCL Standards Complaint: No Evidence to Substantiate | On September 8, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding an allegation of failing to notify the parent immediately of a serious or potentially serious injury. An investigation was begun on September 8, 2023 and concluded on September 19, 2023. There is no evidence to substantiate this complaint for failing to notify a parent immediately of a serious or potentially serious injury because the staff involved stated they checked the child for injury at the time of the incident and did not observe there to be any injury. The staff stated they were not aware of an injury to the child s face until it was pointed out by the parent at pick up time. Staff completed an accident/injury report and provided it to the parent by email after business hours on the date of the incident. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
